Liability Waiver
Release and assumption of risk agreement
By checking the liability waiver box and submitting your registration with Galaxy Soccer Academy Ottawa, you acknowledge and agree to the following terms. Please read carefully before proceeding.
I understand that participation in soccer tryouts and training activities involves inherent risks of injury, including but not limited to cuts, bruises, sprains, fractures, and more serious injuries. I voluntarily assume all risks associated with participation.
I hereby release, waive, and discharge Galaxy Soccer Academy Ottawa (GSA Ottawa), its directors, officers, coaches, staff, volunteers, and facility owners from any and all liability, claims, demands, or causes of action arising from my participation in soccer activities.
I agree to indemnify and hold harmless GSA Ottawa from any claims brought by third parties as a result of my participation in soccer activities.
I authorize GSA Ottawa staff to secure emergency medical treatment for me if needed during activities.
I consent to GSA Ottawa taking and using photographs, videos, and other media of me for promotional, educational, and marketing purposes.
If I am 18 years or older: I am legally competent to enter this agreement
If I am under 18: My parent/guardian must also consent to these terms
By submitting your tryout application, you confirm that you have read, understood, and agree to this liability waiver and all its terms.
